Dr. Cohen and Amanda worked hard to build a busy primary care practice which eventually grew to over 1,000 active patients. Although the parking lot was full, they felt unfulfilled in their purpose and direction. They realized that most of their patients were not getting better by simply masking their problems with medications. A turning point came when it was realized that the care being provided was merely part of the disease management model, which is part of the current broken healthcare system. At that point a decision was made to begin living in their purpose and passion by creating a movement in their community to empower clients to regain control over their health and lives. Declare Wellness Center was created as a new model that focuses on taking a practice approach to health and wellness by addressing root causes of problems rather than just covering up symptoms. Dr. Cohen and his team focus on incorporating five pillars of health into individualized and comprehensive treatment programs designed to get our practice members to their goals. Declare Wellness Center is centered around a "hands on team approach" to health that aims to inspire, encourage, and lead people to a better destination.
